This is what I would do: First, I would convey that the blood of Christ covers all of his/her sin if she would repent and trust in His righteous sacrifice alone for salvation - and even if he/she wasn't able to revert back, his/her salvation would be based solely on the atoning work of Christ. I would make that very clear.

Second, the efforts made to revert back to Biblical Manhood as a fruit of salvation, evidence of a repentant heart, are done in obedience to Christ. Whatever could be done, refraining from hormone therapy, surgical removal of add-ons as money becomes available, should be done in his best efforts, without creating anxiety, but should be done with joy. I think this step(s) would be necessary because if it were any other sinful lifestyle, sexual immorality with a live - in girlfriend, for example, not only would the man have to move out in obedience but the relationship may need to be re-evaluated or even put away also. It is the same in this case. Whatever measures he's doing to maintain false womanhood must be stopped and efforts must be made, in obedience, to become what he was created to be - a man.
